Marisa of Liartop Mountain begins with Reimu's search for her missing friend, a quest that leads the heroine to a mysterious book. This book serves as a portal to a literary world populated by strange inhabitants. Within, Reimu encounters a devastated version of Marisa, openly doubting her own essence and purpose. The main goal is to navigate this symbolic landscape, a mountain of lies reflecting her friend's internal conflict, to uncover the truth and help her return.

The journey through the pages of the book in Marisa of Liartop Mountain is built on choices and dice rolls, simulating a tabletop adventure. Important decisions and the outcome of random events direct the plot along branching paths that converge on several different endings. The player must build a dice inventory by collecting special items with unique properties to manipulate probability. This collection includes dice with high values, intentionally low values, or dice with identical faces, providing strategic tools for influencing the outcome of key plot points.

Throughout the adventure, four other characters offer their opinions as options, offering advice that can be helpful, pointless, or even harmful. Following their recommendations can unlock powerful dice as a reward. The gameplay allows players to spend XP on rerolls to undo unsuccessful results. XP is earned by leveling up, but this makes it more difficult to improve other stats, so the player faces a crucial choice: develop the heroine's skills through stat growth or rely on luck and rerolls.
