Tokyo Underground Killer takes the player into a network of tunnels beneath Tokyo, where Kobayashi uses blood energy to fight enemies. Enemy encounters combine swordplay and supernatural powers, requiring precise timing and planning. Dungeons contain Flatliners, aggressive and fast, forcing the player to reposition and use skills instantly. Objectives focus on eliminating these threats in tight spaces and multi-level areas that affect movement and vision.
