Exorcism Agency tasks top-notch exorcists with entering spirit-infested areas to hunt down entities using mystical techniques. The gameplay combines roguelike card gameplay with a three-element match-3 mechanic, with phantom cards of different colors filling a dynamic battle track. The player must play their cards, eliminating these creatures from the field and quickly completing their objectives.

Creating clever combinations in Exorcism Agency yields generous rewards that can be used to pay for various services in the shop, making each exorcism mission unique. Fights with powerful bosses in the form of ghost girls test the concentration and will of the spirit hunter. When these entities take damage, their health decreases, causing tears in their clothes and loud screams. A character who succumbs to the charms of their opponents risks being completely deprived of life energy.

Victory requires methodically clearing the spirit track using a deck of cards and landing a fatal blow at the right moment. Upon victory, the defeated spirit's embryo is automatically sent to the Ghost Nursery for further cultivation. The Ghost Nursery is a place to nurture captured embryos, turning them into powerful allies. Showing care increases the creature's favor, opening up access to new abilities, battle buffs, and unique collectibles. The player can feed them special offerings earned in battle to speed up the process and unlock features like the camera mode.
