Damned 2 places a group of up to four survivors in a confined location with one goal: escape. They are opposed by a fifth creature that hunts them throughout the area. To achieve this goal, humans must find specific items and solve puzzles scattered around the map. Each session changes the location of keys and components, requiring gamers to adapt in each match.

This system ensures that the path to the exit is different each time the game is played, requiring constant situational awareness. Gamers can choose their role, becoming one of the humans or the monster pursuing them. In Damned 2, the player controlling the creature uses a set of specific abilities to track down and eliminate the others. In turn, the survivors depend on cooperation to effectively complete the challenges of the area.

The computer-controlled creature is also able to hear the voice communication of nearby humans, adding a new layer of interaction between the parties. There are a variety of creatures to choose from, each with a unique style and set of hunting skills. Some of these figures operate stealthily to close in on targets, while others create chaos to disorient opponents. Before a match begins, users can adjust the settings to set their own game difficulty level. The system engages up to five players in a central dynamic of exploration, puzzle solving, and pursuit.
