HOTEL BARCELONA places the player inside a remote structure designed as a trap for American serial killers. The protagonist is Justine, a new federal marshal whose mind is split with the maniac Dr. Carnival. Together, the two personalities must eliminate every criminal within the hotel's walls. Their shared goal is to survive this ordeal and escape the building.

HOTEL BARCELONA takes place in a 2.5D world, where encounters require tactical maneuvers and well-timed attacks. The environment is modeled after American horror films of the 1980s and is fraught with numerous dangers. Each time the character dies, a Slasher Phantom is created an echo of the character's past life. These phantoms repeat precise movements from the previous run, and their actions can damage enemies, acting as bait or as allies in subsequent attempts. Combat activity causes the hero to become covered in blood, a substance that directly powers the power mechanic. An aggressive fighting style fills the Skull Gauge, and once it reaches its maximum, Carnival Awakening can be activated.
