Eliminating opponents is based on the principles of authentic historical European fencing. Encounters with crazed religious fanatics, seasoned mercenaries, and marauders require skillful switching between one-handed and two-handed swords. Exploring the mountain slopes of the Apennines and exploring the ruins of ancient Rome is regularly punctuated by solving intricate spatial puzzles.

The storyline revolves around a young wandering warrior named Aeta, voiced by actress Albie Baldwin. The protagonist desperately attempts to find her missing companion Bianca, voiced by Jennifer English, while simultaneously confronting philosophical dilemmas regarding the justification of brutal methods for a noble cause. Playing 1348 Ex Voto forces you to constantly adapt to the merciless realities of a fading medieval society. To successfully complete a dangerous rescue mission, the girl must:
