Angel Engine is a terrifying surgical simulator whose gameplay revolves around manipulating the bodies of comatose test subjects. Terrifying anomalies are treated remotely using a robotic arm. Confined to an isolated unit, the protagonist fights for the lives of patients under severe time pressure. Each procedure is a complex puzzle, requiring pinpoint precision using specialized instruments. The slightest error can lead to the patient's awakening, which inevitably guarantees a fatal outcome.

In addition to healing deformities, the player must fend off attacks from demonic entities, dividing their attention between monitoring indicators and repairing equipment. The events of Angel Engine unfold on the grounds of the Second Tower of Babel research facility. The surrounding world is a wasteland scorched by atomic fire. Here, the remnants of civilization desperately try to escape the final curse. Playing as an intern, the protagonist gradually delves into the terrifying secrets of the laboratory's leadership. Studying someone else's emails sheds light on the scientists true motives.
